Park Terrace Care Center (PTCC)

PTCC is a 200 bed facility providing 24 hour comprehensive medical, nursing and rehabilitative care. We are a New York State Certified Traumatic Brain Injury Rehabilitation Subacute Program that provides inpatient rehabilitation program for brain injured individuals including coma recovery. The subacute brain injury rehabilitation program provides a link between acute hospital-based TBI treatment and community living. It is a 40-bed intensive rehab, cognitive-focused interdisciplinary program. The length of stay in this level of care ranges from 3-12months. All residents admitted to the program receive 3 hours a day of restorative Physical Therapy (PT), Occupational Therapy (OT), Speech and Language Therapy (ST), and Cognitive rehabilitation as well as ongoing neuropsychological, behavior management and psychological services. For individuals with disorders of consciousness admitted to this program, individualized structured sensory stimulation is provided by nursing as well as incorporated into treatment by OT, PT and ST. In addition, daily structured therapeutic recreation programs are provided for all residents, as well as case management and discharge planning services. 

Job Description:

The speech pathologist will be receiving significant experience in effectively evaluating and treating diverse residents with deficits in speech, language, cognitive-linguistic, disorders of consciousness, swallowing, and tracheostomy-dependent. Additionally, you will maintain records, monitor progress via progress notes, and counsel/educate staff, patients, and their family members.

Responsibilities include:

Provides speech, language, cognitive, dysphagia, and voice therapy to residents.Participates in multidisciplinary consultations and collaborations.Executes a variety of reports including speech evaluations, daily logs, progress reports, recertifications, FIM forms, and letters to outside facilities.Designs, implements and evaluates individualized treatment plans for patients.Plans and utilizes activities to develop and maintain patient's maximum functional ability.Counsels and educates patients, family members, and interdisciplinary staff regarding results of testing, recommendations, and deficits associated with speech, language, cognitive-communicative, and swallowing disorders.Interprets instrumental swallow assessments (MBS and FEES) and ensure appropriate treatment course given results.Engages and participates in research, presentations, and professional conferences, in-service training, clinical staff conferences, and meetings.

Qualifications:

Master's Degree in Speech Pathology, required.1-2 Acute and/or Subacute Experience, required.Knowledge of PC software (Excel, Word), required.Bilingual in Spanish and/or Chinese, strongly preferred (other languages will be considered).Experience with tracheostomy and/or ventilator-dependent patient, preferred.Exposure to Electronic Medical Record (Rehab Optima/Optima Health Care Solutions), preferredAbility to interact with patients, families, medical staff and health system services in a professional and respectable manner.Ability to communicate effectively, both orally and in writing.Please submit your resume to the following email address:SpeechTherapy@Park-Terrace.com
